Wenn Sie per Email Kontakt mit uns aufnehmen oder Sie unser Datenblatt bei einer Erstbesprechung ausgefüllt haben, werden Ihre angegebenen Datenzwecks Bearbeitung der Anfrage und fürden Fall von Anschlussfragen sechs Monate bei uns gespeichert. Wir geben diese nicht ohne Ihre Einwilligung weiter.
Wenn Sie unser Patient geworden sind, (wir also ein Vertragsverhältnis abgeschlossen haben) verwenden wir folgende Daten:
Ihre Personaldaten: akademischer Grad, Sozialversicherungsnummer, Vornahme, Zunahme, Telefonnummer, Geburtsdatum, Email-Adresse, Anschrift,
sonstige Gesundheitsdaten, die Sie uns im Anamneseblatt (siehe weiter unten) bzw. im GROSS MOTOR FUNCTION MEASURE SCORE SHEET zur Verfügung stellen bzw. erhoben werden.
Deletion
We generally delete your personal data and other health data after the termination of the contractual relationship (therapy), but no later than after all legal retention periods have expired. There is the possibility that instead of deletion, the data will be anonymized. In this case, any personal reference to the data will be irreversibly removed, which is why there is no legal obligation to delete data under data protection law.
Data Processing
We process your personal data and permissibly collected health data for the provision of the agreed rehabilitation and therapy services and for invoicing our service (therapy) as well as for processing the agreed rehabilitation and therapy services (processing purpose).
The processing of your personal data is necessary for the conclusion of the contract and for contract processing. The processing is permissible under Art. 6 Para. 1 b of the General Data Protection Regulation. The processing of your health data in the medical history form is carried out for treatment in the healthcare sector and is therefore permissible under Art. 9 Para. 2 lit. f of the General Data Protection Regulation. The processing of health data is carried out exclusively by our physiotherapists as specialist personnel, who are subject to a legal obligation of confidentiality.
If you do not provide us with the data or do not provide it in full, we will not be able to fully fulfill our rehabilitation and therapy services or may not be able to conclude a contract with you.
If you have given us your consent, we will use your personal data for up to three years after the termination of our contractual relationship to provide you with information about our services. We will not transfer your data to third countries or an international organization.
Personal data will only be passed on or transmitted by us to third parties if this is necessary for the purpose of contract processing or for billing purposes, or if you have pre-consented. For example, your data may be disclosed if your health insurance company requests a report on the therapy from us.
Cookies
Our website uses so-called cookies. These are small text files that are stored on your device via your browser. They do not cause any harm. We use cookies to make our offer user-friendly. Some cookies remain stored on your device until you delete them. They enable us to recognize your browser on your next visit. If you do not wish this, you can set up your browser so that it informs you about the setting of cookies and you only allow this in individual cases. Disabling cookies may limit the functionality of our website.
Web Analysis
Our website uses functions of the web analysis service Google Analytics. The provider is Google Inc. This analysis tool also uses cookies to capture standard logging information and visitor behavior on our website in aggregated form. We have enabled the IP anonymization function of Google Analytics by default, which sets the last digits of your IP address to zero. By activating IP anonymization on this website, your IP address will be truncated by Google within member states of the European Union or in other contracting states of the Agreement on the European Economic Area before. The information collected from the visit is usually transferred to a Google server in the USA and stored there. Our concern within the meaning of the GDPR (legitimate interest) is the improvement of our offer and our online presence. Because the privacy of our users is important to us, user data is pseudonymized.
Newsletter
If you have decided to subscribe to the newsletter, we will need your email address and your declaration that you agree to receive the newsletter. In order to provide you with targeted information, we also collect and process voluntarily provided information: first name, last name, salutation. Once you have registered for the newsletter, we will send you an email with a link to confirm your registration. You can unsubscribe from the newsletter at any time. In this case, your data related to the newsletter will be deleted.

This website uses Smart Emailing to send newsletters. The provider is Smart Emailing AG, Neutroufalky 797/5, 62500 Brno, Czech Republic, ID no: 29210372. Smart Emailing AG is a service that can be used to organize and analyze the sending of newsletters. Right to Information: You can request a confirmation from us as to whether and to what extent we are processing your data.

Right to Rectification: If we process your personal data that is incomplete or incorrect, you can request its rectification or completion at any time.
Right to Deletion: You can request the deletion of your personal data from us if we process it unlawfully or if the processing disproportionately interferes with your legitimate protection interests. There may be reasons that oppose immediate deletion (e.g., legally mandated retention obligations).
Right to Restriction of Processing:
You can request the restriction of the processing of your data if
- you dispute the accuracy of the data for a period that allows us to verify the accuracy of the data,
- the processing of the data is unlawful, but you oppose deletion and instead request restriction of data use,
- we no longer need the data for the intended purpose, but you still need this data for the assertion or defense of legal claims,
- you have objected to the processing of the data.

Right to Data Portability: You can request that we provide you with your data, which you have entrusted to us for storage, in a structured, commonly used, and machine-readable format.

Right to Object: If we process data to perform tasks in the public interest, to exercise public authority, or if we rely on the necessity to protect our legitimate interests, you can object to this data processing. The objection is justified if you have a legitimate interest in your data.
Right to Complain: If you believe that we have violated data protection laws in processing your data, please contact us. You also have the option to lodge a complaint with the Austrian Data Protection Authority or, from May 25, 2018, with the supervisory authority within the EU.
If you wish to assert your rights against us, simply use our contact options. If we have doubts about your identity, we may request additional information. Please note that for obviously unfounded or particularly frequently asserted rights, we may charge a reasonable processing fee or refuse to process your request.
CONSENT STATEMENT
For the processing of your personal data that is necessary and required solely for the purpose of executing the contractual relationship, consent is not required under Art. 6(1)(b) of the General Data Protection Regulation. Your health data collected during the anamnesis is necessary for treatment in the health sector and is required for successful therapy. The data processing is permitted under Art. 9(2)(f) of the General Data Protection Regulation.
For any additional collection and processing of your personal data, your consent is generally required. You may voluntarily provide this consent. If you provide consent, please check the relevant boxes. If you do not provide consent, leave the boxes unchecked.
You agree that
- your personal data will be stored for three years for the purpose of sending information about our services,
- your data regarding your habits (eating, drinking, sleeping), family support, and communication for better care and treatment will be stored until the completion of the therapy,
- we may internally evaluate the results of the GROSS MOTOR FUNCTION MEASURE and publish them for scientific purposes (in anonymized form),
- we may use and publish your name, image, and video recordings for advertising and informational purposes.
If the patient is under 14 years of age, the consent must also be given by the legal representatives.
